School Nurse

    Dear Parents/Guardians:

    My name is Stacy Baggstrom. I am the School Nurse at the Charles L. Spragg Elementary School.  I have worked in the district for several years.  I would like to share with you information regarding health and wellness in our schools.     IMMUNIZATIONS/PHYSICALS: Have your child’s immunization record and physical up to date. The State of New Jersey requires that a child MUST be kept home if his/her shots are not current or if a physical from the physician is not on file.

    MEDICATIONS AT SCHOOL: Medications can be given at school if needed. Forms are available in the Health Office. Students are not allowed to take medication of any kind in school without a physician’s order. This includes over-the-counter medication. A licensed physician must issue a doctor’s order. The written orders must include the diagnosis or type of illness, the name of the drug, dosage, and time of administration. The medication should be brought to the school in its original container, appropriately labeled by the pharmacy. Medication is never to be sent in by your child. The parent/guardian must bring in the medication and it will only be accepted and signed in by the School Nurse. ** Every new school year you have to renew the medication order by the physician.

    ASTHMA: If your child has asthma, they should have medication available at school in case of an attack. Precious moments are wasted and could be life threatening if your child does not have asthma medication at school. Forms for you and your doctor to fill out (Asthma Action Plan) are available in the Health Office. If we do not have the proper documentation from the physician and the medication on hand, we will have to call 911 in the event of an emergency. (Form can be printed from bottom link)

    ALLERGIES: If your child has any food allergies, the State of New Jersey requires our food service department to have proper documentation from your child’s physician in order to accommodate them for breakfast and/or lunch.

    MEDICAL HEALTH QUESTIONNAIRE: These forms are sent home on the first day of school. The forms must be filled out by parents/ guardians within the first week. It is very important that you send them back because it gives the nurse an update of your child’s current medical status, numbers you can be reached at in case of an emergency, and consent to perform yearly annual screenings. If your phone number changes any time during the year, please call the office so we can make these changes.

    SENDING SICK CHILDREN TO SCHOOL: Please do not send your child to school if he/she has vomited or had diarrhea within 24 hours, fever greater than 99.6 F, continuous cough, skin rash or a red, itchy, draining eye. If your child is sent home with a fever, vomiting or diarrhea, your child must stay home for 24 hours. If your child is sent home for an unidentified rash or draining eyes, they need a doctor’s note to return to school.

    YEARLY SCREENINGS: Height, weight, blood pressure, vision, and hearing are completed throughout the year. Depending on the age of your child, they may have all of these screenings or some of them done annually.  If your child fails a screening or has a discrepancy in weight or blood pressure, you will receive a notice with suggested follow-up. Please note this is a screening ONLY, not a diagnosis.

    ****FLU SHOTS are available during flu season at the Pediatrician Offices.

    Any student attending school that is 59 months and younger is required by NJ State Law to receive 1 dose of the Flu Vaccine annually.

    ****BIKE HELMET SAFETY Children under 17 years old are required to wear helmets when riding a bike, skating, and skateboarding.
